This is a song for Eucharist and with nine verses covers just about all liturgical bases.
The tune is by Brett Ballard. The text is by Bernard Geoffroy (? translated) but it appears he was assisted for this version by Alan Hommerding on verse 2 and Brett Ballard on verses 6-9. There are other versions and other settings with fewer verses.

It is an excellent mainstream communion hymn that as you say , covers all bases, (an easier to sing version of Michael Joncas’s “Take and Eat” in Gather Australia!) and we use the version in Gather 3 regularly.
